FSSAI License Checklist
According to the Food Safety and Standards Act, an FSSAI registration is mandatory for all food business operators. This registration must be obtained through the FSSAI Food Licensing and Registration System. When applying for this license, keep a document checklist handy for ease.
Documents Required for FSSAI Registration
Here are the documents to be compulsorily submitted:
Application in Form A signed by the proprietor/authorised signatory of the enterprise
Certificate of incorporation or partnership deed or memorandum and articles of association
Photo Identity of Food Business Operator (FBO) i.e. of the proprietor/authorised signatory of the enterprise such as Voter ID card, driving license, passport, PAN card, Aadhar card
Specific details about the food products which the enterprise produces
Management plan in place for food and health safety
Proof of office and factory/unit premises occupancy, whether possession certificate or rental agreement
Any other supporting documents such as a No Objection Certificate (NOC) issued by the local health department, panchayat or municipality
Where to File
The FSSAI registration may be initiated online by submitting an application in Form A to the Food Safety and Standards Authority of India on their website. The department analyses the application and supporting documents. The final decision about granting registration is communicated to the applicant within seven days from the online filing date in writing. If the application has been accepted, the FSSAI will grant a certificate of registration which states the registration number and displays the photo of the applicant.
Things to Remember
A couple of important things to remember. It is mandatory for an FBO to display the FSSAI certificate prominently at the place of business. All small businesses in the food sector need to mandatorily apply for this basic FSSAI registration.
Documents Required for License Application
For the FSSAI Central License
Application in Form B signed by the proprietor/authorised signatory of the enterprise
Personal details of authorised signatories whether proprietor/partners/directors along with Photo Identity such as Voter ID card, driving license, passport, PAN card, Aadhar card and contact details
Trade license/partnership deed/memorandum and articles of association
Proof of office and factory/unit premises occupancy, whether possession certificate or rental agreement
Specific details of food products which the enterprise produces
Sources of raw materials to be used such as meat, milk and so on
Water analysis report which confirms the water potability
Detailed plan of the unit specifying size and area allocation for each operation
List of machinery and equipment to be used along with name, make and model, capacity and numbers installed of each
The plan in place to recall food products manufactured from the market
In case the unit is export-oriented, a certificate from the Ministry of Commerce in this regard
IE code issued by the Director General of Foreign Trade
Management plan in place for food and health safety
Any other supporting documents such as a No Objection Certificate (NOC) issued by the local health department, panchayat or municipality
Form IX
Ministry of Tourism certificate (in certain cases)
Documents supporting period wise turnover figures and transportation of products
Self-declaration form
For the State License
Application in Form B signed by the proprietor/authorised signatory of the enterprise
Personal details of authorised signatories whether proprietor/partners/directors along with Photo Identity such as Voter ID card, driving license, passport, PAN card, Aadhar card and contact details
Trade license/partnership deed/memorandum and articles of association

Things to Remember
For medium and large scale FBO businesses, it is mandatory to obtain an FSSAI license. Medium scale FBOs such as manufacturing units, marketers, transporters, traders and so on must also apply to the state government. Large scale FBOs such as manufacturers, exporters, importers and so on must apply to the central government.
